Dear Parents and Guardians,

This is a friendly reminder that all AAP Level IV referrals are due by the close of business on Friday, January 15, 2016. If you wish your child to be screened for Level IV Services this year, at minimum you must submit a referral which is signed and dated. Please submit all items together in an envelope to the FHES Office addressed to me. I will then send you an e-mail acknowledging receipt.
